Skilled Nursing (RN Visits)

Skilled nursing provides registered nurse (RN) visits in the home to support recovery, monitor symptoms, and help you understand your plan of care. Visits are provided based on physician orders and scheduled according to clinical need.
Who skilled nursing can help
RN visits may be appropriate for adults who need skilled support at home, such as:
- After a hospital stay, surgery, or a recent change in condition
- When new medications require monitoring and education
- When symptoms need assessment (for example: swelling, shortness of breath, dizziness, pain changes)
- When home safety and care routines need reinforcement
- When multiple providers/services need coordination
We’ll help confirm whether skilled home health is appropriate and what documentation may be needed. We do not promise specific medical outcomes.
What RN visits may include
Assessment & monitoring
- Health assessments and symptom monitoring
- Vital signs as appropriate
- Recognizing and communicating changes to the ordering provider
- Safety check-ins to support care at home
Medication support
- Medication review and education (as ordered)
- Side-effect and safety teaching
- Help organizing routines and understanding instructions
- Coordination with your provider/pharmacy when needed
Care plan reinforcement
- Teaching on your plan of care in plain language
- Condition-specific education (as ordered)
- Support for caregiver understanding and confidence
- Follow-through on provider instructions
Coordination & communication
- Updates to the ordering provider as needed
- Coordination with therapy/other disciplines (when part of the plan)
- Documentation aligned with physician orders
- Clear communication with patients and families
Services provided depend on physician orders and your individualized plan of care.

- RN visits begin: Visits are scheduled based on clinical need and the physician-ordered plan of care.

- A list of current medications (or the bottles/containers)
- Recent discharge paperwork or visit summary (if available)
- Your primary care provider and specialist names
- Questions you want answered
- A clear space to sit and talk (kitchen table works great)
