In-home wound care provides skilled assessment and dressing care based on your physician’s orders. We focus on safe routines, clear education, and coordinated communication with your ordering provider—so you understand what to do at home and when to reach out.
Who wound care can help
In-home wound care may be appropriate for adults who need skilled support at home for wounds or skin concerns, such as:
- After surgery (incision support as ordered)
- Skin tears or wounds that require dressing care per plan
- Wounds that need ongoing assessment and documentation
- Patients who benefit from education on safe routines and warning signs
- Caregivers who want clear guidance on how to help at home
Services depend on physician orders and your individualized plan of care. We do not promise specific outcomes.
What wound care may include
Assessment & documentation
- Wound/skin assessment per plan of care
- Measurement and documentation as required
- Monitor for changes and report concerns to the ordering provider
- Reinforce home safety and care routines
Dressing care (as ordered)
- Dressing changes based on physician orders
- Skin integrity support and protective routines
- Education on keeping the area clean and protected
- Comfort-focused tips within plan of care
Education for home routines
- What to do between visits (in plain language)
- How to reduce risk of irritation or breakdown
- How to recognize changes that need a call
- Caregiver teaching when appropriate
Coordination & communication
- Updates to the ordering provider as needed
- Coordination with nursing/therapy as part of the plan
- Documentation aligned with physician orders
- Clear communication with patients and families
What’s provided depends on physician orders and your plan of care.
When to call (warning signs)
If you notice any of the following, contact your provider or call us so we can help route the concern appropriately:
- New or worsening redness, warmth, swelling, or increasing pain
- Drainage that is new, heavier than usual, or has a strong odor
- Fever or chills
- The dressing will not stay in place or becomes saturated quickly
- Any sudden change that concerns you
Emergency: If there is uncontrolled bleeding, severe symptoms, or you feel unsafe, call 911.
What to expect
- Reach out: Call or request a call. We’ll confirm service area and basic needs.
- Confirm next steps: If appropriate, we’ll explain what’s needed (physician orders and coverage requirements may apply).
- Visits begin: Visits are scheduled based on clinical need and the physician-ordered plan of care.
Please don’t include sensitive medical details in online forms. We’ll collect the right information securely when we speak.
What to prepare (helpful)
- Your discharge paperwork or wound care instructions (if available)
- A list of current medications (or the bottles/containers)
- Any wound supplies you have at home (if provided)
- Questions you want answered
- A clean, well-lit space to set up for the visit
